Additional Agent Instructions
To expedite the Qualification process, it is best to submit all
supporting qualification documents at one time.

NAVL Drug Test: All drivers must have a negative Pre-Employment (PE) drug test on file with the van lines. The drug test results are valid for only 30 days from date of test.
Each agent should have a supply of their North American Federal Chain of Custody (drug test)
forms (Sample Form).
If you do not have these forms, please contact Lori Hart @ lori.hart@sirva.com or 260-429-3302.
You will need to give the driver the drug test form and direct him/her to an NAVL approved clinic
(Drug Test Sites).
The NAVL drug test form is not on file with the clinics.
Canadian Agents please contact Lori Hart for specific instructions.

Driver Relationship Form: This form must be completed by an Agent Representative. Those requiring ELDs must be set-up in Driver Connect prior to first trip.

Record of Road Test: Road Test are to be given in the type of vehicle he/she will be operating. The Road Test Examiner must be a qualified Allied or North American driver and licensed for the appropriate equipment.

Proof of Insurance Coverage: Proof of Insurance Coverage must be submitted to and approved by the Insurance Compliance Department,
for any type of driver other than an agent employee,
before the driver will be qualified
(Insurance contact information).

Independent Contractor Operating Agreement (ICOA):
Any owner operator or lease operator must have an approved ICOA on file with the van lines before being placed into service.

Safety Knowledge Test and CSA2010 Course:
Once a Safety Number is issued, the driver will be required to take and pass these online tests/courses found by logging into
Luma Driver Training. The driver must enter his/her Safety Number and the password welcome for their first time logging in.
These requirements must be completed within 30 days after being issued a Safety Number.
